Pulmonary Embolism Rule-Out Criteria

Rules out PE without further testing when ALL 8 criteria are absent and pre-test probability is low (<15%). Sensitivity >97%.

Age ≥ 50 years?
Heart rate ≥ 100 bpm?
SpO₂ < 95% on room air?
Unilateral leg swelling?
Haemoptysis?
Recent surgery or trauma (requiring general anaesthesia within 4 weeks)?
Prior DVT or PE?
Oestrogen use (OCP, HRT, or hormone-containing contraceptives)?

PERC Criteria Met/ 8
PERC Negative — PE can be excluded if pre-test probability is low
All 8 PERC criteria absent. In low pre-test probability patients (Wells PE ≤1 or clinical gestalt <15%), this rules out PE. No D-dimer or imaging required. Do not apply PERC to patients with intermediate or high clinical suspicion — proceed directly to D-dimer or CTPA in those cases.
