Bleeding Risk Score in Atrial Fibrillation

Estimates 1-year major bleeding risk in patients on anticoagulation for AF. Does not contraindicate anticoagulation.

Uncontrolled hypertension — SBP > 160 mmHg
Abnormal renal function — dialysis, transplant, or creatinine > 200 µmol/L
Abnormal liver function — cirrhosis, or bilirubin > 2× / transaminases > 3× ULN
Prior stroke history
Prior major bleeding or predisposition to bleeding
Labile INR — unstable/high INR, or TTR < 60%
Elderly — age > 65 years
Drugs — antiplatelet agents or regular NSAIDs
Alcohol excess — ≥ 8 drinks per week
