Edmonton Frailty Scale

Multidomain frailty assessment for pre-operative and medical patients. Score ≥8 indicates frailty requiring targeted interventions.

Number of prescription medications taken regularly
Cognitive function (Clock Drawing Test result)
General health status
Functional independence — IADLs (finances, transport, housework, medications, phone, meals)
Do you feel that you lack social support when you need it?
Do you sometimes forget to take your medications or take the wrong dose?
Have you lost >5 kg of body weight unintentionally in the past 3 months?
Do you often feel sad or depressed?
Do you have a problem with bladder control / urinary leakage?
Timed Up and Go (TUG) test result (or clinical equivalent)
