Abbreviated Mental Test Score

Rapid 10-question bedside cognitive screen. Score ≤7 suggests cognitive impairment requiring further evaluation. Takes ~2 minutes to administer.

Patient correctly states their age?
Patient correctly states their date of birth (day and month)?
Patient correctly states the current year?
Patient correctly states the current time (within 1 hour)?
Patient correctly identifies the type of place they are in (e.g. hospital)?
Patient can recall a 5-component address given at the start (e.g. "42 West Street, Grantham")?
Patient can recognise two named persons (e.g. doctor and nurse)?
Patient knows the year of a notable historical event (e.g. WW1 start: 1914)?
Patient can name the current head of state or monarch?
Patient can count backwards from 20 to 1 without error?

Score 0–4: Significant impairment. Urgent assessment for delirium or dementia. Collateral history, safety review, and specialist referral indicated.
