Ranson Criteria for Acute Pancreatitis Severity (Non-Gallstone / Alcoholic)

Predicts severity and mortality of acute pancreatitis using 5 admission criteria and 6 criteria assessed at 48 hours.

Admission: Age > 55 years
Admission: WBC > 16,000/µL
Admission: Blood glucose > 11.1 mmol/L (> 200 mg/dL)
Admission: Serum LDH > 350 IU/L
Admission: AST > 250 IU/L
48 h: Haematocrit fall > 10 percentage points
48 h: BUN rise > 1.8 mmol/L (> 5 mg/dL)
48 h: Serum calcium < 2.0 mmol/L (< 8 mg/dL)
48 h: PaO₂ < 60 mmHg
48 h: Base deficit > 4 mEq/L
48 h: Estimated fluid sequestration > 6 L

Mild acute pancreatitis
< 1% mortality. Score 0–2: mild disease. Conservative management with IV fluids, analgesia, and NPO. Monitor closely for deterioration.
