ACR/EULAR Gout Classification Criteria 2015

Classifies gout in patients with ≥1 episode of peripheral joint/bursa swelling, pain, or tenderness. Requires ≥8 points to classify as gout (sensitivity 92%, specificity 89%).

Monosodium urate (MSU) crystals identified in synovial fluid or suspected tophus (definitive criterion)?
Synovial fluid analysis PERFORMED and NO MSU crystals found?
Affected joint/bursa site during symptomatic episode
Episode characteristics (erythema, can't bear touch/pressure, great difficulty walking/using joint)
Episode pattern
Serum urate (measured during attack-free period if possible)
Imaging showing urate deposition (dual-energy CT, ultrasound double-contour sign, or tophus on CT)?
X-ray showing gout-related erosion (NOT OA — cortical erosion with sclerotic margin)?
