- Severe acute or symptomatic hypocalcaemia
- Acute severe hyperkalaemia [plasma-potassium concentration 6.5 mmol/litre or greater, or in the presence of ECG changes]
- Calcium deficiency, Mild asymptomatic hypocalcaemia
